Building Multicultural Communities (BMC) is a charity established in 1987 that provides settlement and social services to newcomers, immigrants and the larger community in the Peel Region. Our mandate and core business is to enable less advantaged individuals, families and groups in the community to reach their full potential, enhance their life skills, and create a better future. BMC has a variety of specialized programs that cater to specific demographics and focus on providing different services such as (Settlement, Mental Health, Youth Development, Employment, and Seniors Support) making BMC a one-stop-shop for needed services. As a Case Manager / Facilitator, you will provides one‑to‑one mentorship‑style support; tracks progress and assessments; and helps run logistics, referrals and pitch preparation. Co-delivers workshops while serving as the primary liaison with Probation/Parole to ensure attendance tracking, documentation, and alignment with supervision expectations.
RESPONSIBILITIES
- Plan and deliver interactive workshops (36 annually) tailored to participant needs; integrate cognitive‑behavioural skills and pro‑social development (communication, emotional regulation, problem‑solving, conflict resolution).
- Provide weekly one‑to‑one support to build trust, address barriers and keep participants on track with individualized plans.
- Administer pre/post assessments, attendance tracking and participant feedback tools; maintain detailed case notes in a timely manner.
- Support logistics (participant reminders, incentives distribution, materials, guest speakers) and pitch showcase preparation.
- Identify risk factors, crises or safety concerns promptly and escalate to the Program Manager. Serve as the primary liaison with Probation and Parole Officers regarding attendance, engagement, compliance updates, and program completion status participate in case conferences as required
- Encourage and facilitate referrals to wraparound supports (housing, mental health, addictions, education, employment).
- 12 workshops delivered per cohort with >80% participant satisfaction (as measured by post‑session feedback).
- Complete attendance and assessment records for all participants; timely contribution to quarterly reporting.
- Documented one‑to‑one supports for each participant and progress against individualized goals.
- Collaborate with Program Manager; Mentorship Coordinator; mentors; admin support.
- Collaborate with guest speakers; entrepreneurship partners; referrals to housing, health and employment providers; Probation/Parole as needed
- Attend staff meetings and training sessions as required
- Any other duties related to the settlement services and this position
- Reports to Executive Director or his/her designate
QUALIFICATIONS/SKILLS
- Degree or diploma in Social Work, Community Services, Adult Education or related field.
- 2+ years’ experience in group facilitation and one‑to‑one case management with justice‑involved or high‑risk youth/young adults.
- Demonstrated ability to deliver CBT‑informed skills training and trauma‑informed, culturally responsive programming.
- Strong documentation, communication and engagement skills with diverse communities
- Demonstrated skills in case management, assessment intervention, and advocacy
- Ability to multitask while maintaining high level of accuracy in data collection
- Excellent interpersonal, organizational, communication, presentation and facilitation skills
- Ability to work with diverse people and communities with awareness to socio-economic and cultural barriers
- Capability to handle multiple priorities with ease and professionalism; able to work in a fast-paced environment with minimal supervision
- Strong communication (oral and written), and computer skills (Word, Excel, PowerPoint, Outlook), Social Media (Facebook, Twitter, LinkedIn etc.) and Web based databases.
- Clear Vulnerable Sector Police Check prior to start.
- Must have own transposition, driver license and insurance to use your vehicle for job related travelling
- Ability to work some evenings/weekends; local travel between delivery sites.
Location:
Hamilton and Halton Region including itinerant community sites (primary delivery)
