DUTIES

• Work collaboratively with other service team members, placement students, volunteers, and families to deliver client-centered, high-quality care support services.
• Research and develop age appropriate, safe and respectful programs to connect the elderly with digital technology to stimulate cognitive function and exercise.
• Plan, facilitate and evaluate research informed and evidence-based activities to support elderly persons living with Alzheimer’s Disease and related Dementia.
• Conduct community outreach and promote digital literacy to the elderly in ethnocultural communities especially the Italian, Spanish and Portuguese Community.
• Ensure client’s dignity, privacy and rights are well respected and protected in the program.
• Maintain confidentiality of client’s information and participation records, document learning outcome and report progress of participants.
• Support the implementation of health and safety regulations in accordance with program requirements and organizational guidelines.
• Ensure the safety of clients by adhering to Health & Safety regulations and policies and be alert to safety concerns and respond promptly.
• Participate in team meetings and attend case management meetings with family and clients.
• And perform other duties as assigned.

QUALIFICATIONS

• Bachelor of Social Work.
• Minimum of 1 year experience in providing supportive care to seniors in community setting.
• Minimum of 1 year experience working with individuals living with dementia.
• Must be fluent in either Italian, Spanish or Portuguese.
• Demonstrated ability in developing, implementing, and evaluating recreational programming/activities that will contribute to health and wellbeing of program participants.
• Functional knowledge of Microsoft Office Suite, Internet, Excel, and online platforms is essential.
• Demonstrated ability to lead a support team to plan and undertake full range of activities for seniors diagnosed with dementia.
• Must have valid First Aid and CPR training certificate
• Demonstrated ability to write program delivery reports
• Demonstrated ability to independently work, make decisions, and respond to multiple priorities and to meet deadlines successfully under pressure.
• Excellent verbal and written communication skills.
• Must be fully vaccinated or be exempt from vaccination on human rights grounds. COSTI reserves the right to request information establishing the validity of the exemption.

The position is open to Union members on a secondment basis.

LOCATION: 2115 Finch Ave W, Toronto
DURATION: September 3, 2024 to March 31, 2025 with a possibility of extension
SALARY RANGE: $ 29.56 - $ 30.99 per hour
