Contract to March 31st, 2027, with possibility for renewal. Salary $37.68 per hour and benefits
The Canadian Centre for Victims of Torture (CCVT) is looking for an experienced person to fill the position of a Case Management Mental Health Counselor (CMMC). The CMMC is part of a flexible and multi-disciplinary team who deliver services and enables survivors impacted by violence to regain their independence and quality of life with their families. The CMMC is responsible for providing trauma-specific mental health Case Management along with assessment and referrals, and public education.
Specific responsibilities include:
- Provides trauma informed case management service to clients including intake and assessment, direct supports, referral &client accompaniment and Indirect supports such as Resources and Training and engaging non=sector partners.
- Facilitates support groups, including planning group sessions, inviting clients, guest speakers, monitoring and evaluating sessions
- Collects and maintain records (e.g. clients and community resources) relevant to the program. Input all activities/services provided into OCMS and iCARE or any other data base the agency makes available
- Provides monthly statistical and narrative reports to Manager, and to conduct evaluations as needed.
- Attends CCVT staff meetings and professional staff development opportunities deemed necessary and appropriate by the manager
- Participate in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, groups.
- Work closely with other settlement and trauma counselors and external partner organizations
- Provide client services at CCVT branch offices, satellite locations, in addition to main office and at partner agencies and
- Prepare and deliver public education presentations
- Prepare reports on volume and effectiveness of client services as per funder requirements
- Performs outreach to potential clients in the catchment area
- Liaises with appropriate community agencies in order to link clients to services relevant to their needs
Required Qualifications:
- MSW degree with one-year experience or BSW with three-year experience or equivalent education in Health Services
- Excellent case management, engagement and assessment skills
- Commitment to working within an anti-oppressive framework and demonstrated understanding of the links between trauma and mental health, emotional health, and substance use problems
- Excellent knowledge of relevant community resources
- Ability to work effectively in physical offices or virtual environments
- Excellent interpersonal and communication skills.
- Extensive knowledge and experience working with the refugee population
- Fluency (written and spoken) in second language. French is an asset
