Are you passionate about helping people navigate complex systems and access the support they need? Community Connection is seeking a Community Navigator to join our Weekday Team. This on-site role is part of our Systems Navigation Program, which bridges gaps in service access through coordinated support, advocacy, and timely referrals. As a Community Navigator, you’ll engage with clients across multiple channels—phone, text, chat, email, walk-ins, and in-person outreach (various service locations), providing trauma-informed, culturally competent support that makes a real difference.
Key Responsibilities
- Provide empathetic, professional client service across all communication channels
- Assess client needs and identify appropriate services and resources
- Conduct screenings, assist with applications, and make referrals
- Advocate for clients navigating complex systems (e.g., housing, healthcare)
- Respond to urgent situations with crisis intervention and safety planning
- Document interactions and maintain data integrity in client management systems
- Collaborate with service providers and contribute to shared service models
Qualifications & Job Requirements
- Post-secondary diploma or degree in mental health, social work, health, or related field
- Minimum 3 years of experience in office and team environments
- Experience in case management and trauma-informed practices
- Required to work on-site for all scheduled shifts
- Required to travel to other service locations for in-person service delivery
- Valid Driver's License, and access to a reliable vehicle
- Acceptable Vulnerable Sector Check
