Building Multicultural Communities (BMC) is a charity established in 1987 that provides settlement and social services to newcomers, immigrants and the larger community in the Peel Region. Our mandate and core business is to enable less advantaged individuals, families and groups in the community to reach their full potential, enhance their life skills, and create a better future. BMC has a variety of specialized programs that cater to specific demographics and focus on providing different services such as (Settlement, Mental Health, Youth Development, Employment, and Seniors Support) making BMC a one-stop-shop for needed services. As a Case Management Counsellor you will provide personalized, intensive, and client-centered case management services for individuals and families with complex needs. This role involves conducting assessments, creating individualized action plans, coordinating referrals, and ensuring clients can access a wide range of services and support that enhance their capacity towards greater independence and integration.
RESPONSIBILITIES
- Conduct comprehensive intake assessments (initial, ongoing, and exit) for clients referred to case management.
- Develop individualized action plans with short-term and long-term goals based on client needs.
- Provide direct support to clients, including navigation of services, appointment accompaniment, and adaptation to life in Canada.
- Connect clients with IRCC-funded and non-IRCC services such as housing, employment, education, healthcare, and mental health supports.
- Offer crisis intervention and support for clients experiencing urgent challenges such as homelessness, violence, or mental health crises.
- Organize and facilitate workshops or sessions on settlement, integration, and community resources.
- Maintain accurate and confidential client records, prepare reports, and meet program evaluation requirements.
- Track client progress through midpoint and exit assessments, ensuring goals are met.
- Ensure proper documentation and reporting through BMC database(s) and IRCC iCARE system.
- Participate in outreach and networking activities to strengthen program visibility and partnerships.
- Maintain strong relationships with community partners to ensure seamless referral and service coordination, building relationships with local shelters, food banks, and legal aid organizations.
- Attend staff meetings, training sessions, and professional development opportunities.
- Perform other duties as assigned related to case management and settlement services
- Accompany clients to scheduled appointments as needed
- Reports to Executive Director or his/her designate
QUALIFICATIONS/SKILLS
- Post-secondary education: Degree/Diploma in Social Work or Social Services
- Minimum of two (2) years relevant experience in settlement sector with proven experience in assisting newcomers with settlement issues.
- Extensive knowledge of the immigration system and community services, programs, resources and outreach practices. Strong understanding of issues facing immigrants/newcomers in Peel and Halton regions.
- Ability to work independently and co-operatively and a team player
- Extensive knowledge of the immigration system, and services available to assist newcomers, refugees and their families.
- Demonstrated skills in case management, assessment and intervention, advocacy
- Ability to multitask while maintaining high level of accuracy in data collection
- Ability to work with diverse people and communities with awareness to socio-economic and cultural barriers
- Excellent interpersonal, organizational, communication, presentation and facilitation skills
- Strong leadership skills to guide, motivate and inspire program participants to take action to successfully complete the program
- Capability to handle multiple priorities with ease and professionalism; able to work in a fast-paced environment with minimal supervision
- Strong computer skills (MS Word, Excel, PowerPoint, and Office 365), Adobe software and web based databases (iCARE/CTIMS).
- Must be fluent in English; the ability to speak, read, and write in Hindi, Punjabi, Urdu, Arabic, Farsi, or Spanish is an asset.
- Adaptive to flexible working hours including evenings and weekends as needed
- Police Record Check for Vulnerable Sector is required as part of the hiring process
- Must have own transposition, driver license and insurance to use your vehicle for job related travelling.
- BMC case management program supports the Peel and Halton Region, all candidates must be able to travel for various itinerant service locations, including accompanying clients for service delivery appointments.
