Summary

Position Summary: Under the general guidance of the Manager – Health Promotion Programs, the incumbent will provide day-to-day leadership to the Locally Driven Population Health Models project (LDPHM) and support the delivery of culturally specific community outreach, education and information. The incumbent will also promote awareness of community health initiatives within the Peel community, ensuring alignment with public health guidelines and the overall goals of the LDPHM project. Compensation: $29.00 per hour plus benefits

Responsibilities and Duties: (but not limited to)

  • Assist in the planning, developing, implementing, evaluating and coordination of the LDPHM project.
  • Share health promotion information with community members and local businesses.
  • Organize health promotion sessions, and distribute resources through tabling sessions, events and presentations at community venues, religious venues, and businesses.
  • Respond to community inquiries on health promotion topics, offering strategies and resources to support community wellness and education.
  • Conduct intake and facilitate referrals to internal and external partners, professionals, services, or community resources based on the community’s needs using a case-based management approach.
  • Collect and report daily outreach data from outreach events and activities.
  • Coordinate with Health Common Solution Labs, community & funder resources as required.
  • Lead data collection and analysis for the LDPHM project ensuring accuracy and providing key insights to the Manager.
  • Coordinate community supports and distribution of other wraparound support services.
  • Assist in the development and delivery of various health promotion programs/initiatives within the community and internal programs including culturally responsive outreach, education, and health promotion initiatives.
  • Assist with grant/funding applications.
  • Inform and educate community members on public health measures, reinforcing provincial and regional health guidelines.
  • Dispel culturally specific myths and misinformation through tailored education and information sessions.
  • Promote vaccination and other health related programs.
  • Refer community members to appropriate resources and supports within the community.
  • Maintain accurate record-keeping/documentation, statistics and administrative tasks as required by the funder.
  • Participate in staff and team meetings, and professional development opportunities as required.
  • Adopt safe work practices, reporting unsafe conditions immediately.
  • Perform other duties as required.

Qualifications and Skills: 

  • Post-secondary education in a relevant discipline and a minimum of three years’ experience in health promotion, health services and/or outreach in a community setting is required.
  • Management/supervisory experience including administration, management and organization, program development, personnel management preferably in the non-profit, community services or health sector is required.
  • Strong knowledge and understanding of community resources of the Region of Peel.
  • Excellent communication and presentation skills to clearly convey information in a manner that engages the audience and helps them understand and retain messaging.
  • Knowledge of and experience with group dynamics and group facilitation approaches.
  • Experience in program development, implementation, monitoring, and evaluation for programs of a multi-disciplinary nature, including experience with statistical analysis (quantitative and qualitative).
  • Strong critical analysis, problem-solving, planning, decision-making, leadership, organizational, and change management skills.
  • A strong team player, with demonstrated leadership skills to mentor, guide and support personnel under stressful conditions
  • Strong oral and written communication skills required and the ability to communicate in one or more South Asian languages is an asset.
  • Advanced proficiency in MS Office, email, and other applications.
  • Must be able to work a variety of shifts including evenings and from a variety of locations within the Region of Peel.
  • Demonstrate a strong interest in making an impact in the community through service, leadership, and education.
  • Ability to mobilize people at the grassroots; strong networking skills.
  • A high degree of conflict management to dealing effectively with others in hostile situations; using appropriate interpersonal styles and methods to reduce tension or conflict.
  • Sensitivity to people from different socio-economic backgrounds with an equity lens.
  • Self-directed, self-motivated, with a positive attitude and be able to work independently and as part of a team.
  • Ability to prioritize, multi-task and adjust to shifting priorities.
  • Valid driver's license and access to a reliable vehicle is required.
  • Successful candidates must provide a satisfactory Vulnerable Sector Check prior to commencing employment.
