POSITION: Prevention Specialist

REPORTS TO: Community Health Program Manager

WORK HOURS: 35 hours per week (Immediate Start)

RATE OF PAY: $27 per hour

JOB SUMMARY:

This position entails providing preventive care support to individuals and families in Mississauga, focusing on chronic disease prevention and addressing social determinants of health. We seek a dedicated Prevention Specialist who is committed to strength-based care and passionate about collaborative work to reduce barriers and improve access to community resources and comprehensive healthcare.

 DUTIES AND RESPONSIBILITIES:

  • Provide preventive care support to individuals and families, including education on chronic disease risk factors and preventive measures
  • Conduct comprehensive needs assessments and develop personalized Prevention Plans in collaboration with participants
  • Facilitate goal setting and action planning using motivational interviewing and behavior change techniques to encourage healthy behaviors (e.g., healthy eating, physical activity, and smoking cessation)
  • Connect participants to relevant community resources and preventive care programs, and support service navigation
  • Conduct follow-up sessions (e.g., at 3- or 6-months) to monitor progress on Prevention Plans and adjust as needed
  • Collaborate with healthcare providers and community partners to ensure a connected, culturally responsive approach to preventive care
  • Educate and support community members on chronic disease prevention, including healthy behaviors and screenings
  • Maintain accurate records of Prevention Plans, referrals, and participant progress to support reporting and program evaluation

QUALIFICATIONS:

  • University degree in a health-related field (e.g., health promotion, public health) or relevant certification
  • Minimum two years of experience in preventive care, community health, or related fields
  • Experience with chronic disease prevention and behavior change techniques
  • Proficiency in Arabic language is an asset
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong problem-solving abilities and the capacity to manage multiple tasks and deadlines
  • Ability to adapt to changes and work collaboratively with diverse teams
